We were early for our appointment. We got in right away and Chelsea Ethan saw us before our appointment time. It was awesome. I would recommend this place to everyone I know.

I wish I could give more stars to this astablishment they actually care for you and they stick to ur health plan that they make for you and Dr shabandar is the best hes not like other Dr's he is down to earth been going to him since 2023 and I wouldn't change it for the world they deserve like 2million stars in my opinion

I've had many procedures done and have been very pleased with results Everyone is so kind and helpful. I came here many years ago and didn't like the Dr I had then whose not here now. I returned and can say I recommend it. Only issue sometimes is the long wait times but understand it happens sometimes. I will continue coming here because my quality of life has improved and am treated respectfully.

I've been going here for sometime and Dr Simon Ho is a great Dr if you have disc issues I met him when I moved here from chgo and he's helped me stay away from back surgery ty so much

I always need cortisone shots in my back and hips. I don't get sedation, and the Dr. Tells me what is about to happen. I've been seeing them for years

From the first phone call from the clinic they accused me of a drug addict when I was referred by a Dr i reluctantly showed for the appointment. The nurse practitioner started to say that everything my doctor diagnosed mind. You nurse practitioner, doctor she debunked and said nothing was there no discolorations, no, nothing still no apology for drug addiction, accusing to my opinion. Doctor perank's office needs to be closed down.My boyfriend went years ago and he put him through drug withdrawals, because he went from the balco clinic to the Laporte clinic and didn't even put him on a reduced narcotic, just shut him off completely.So tell me how good your doctor's office is

Was the worst time of my life i thought that doctors were to help you not just look the other way. This lady was my doctor for many years before she started there she feed me all these pills like they were nothing. Now they refuse to help me. If this is not a place i would tell anyone to go to.

Despite the note to refill my pain meds at a quantity to hold me over til the appt scheduled, it was never sent to my pharmacy. Therefore O had to enduren3 days with only Aleve to help me. I lost much beneficial sleep those 3 nights and could barely move dou to renewed fibromyalgia pain setting in unhindered. Still, a day after my appt I am still waiting for my meds. I hope this doesn't happen to anyone else.

Me and my wife has 2 dogs so of course they are going to climb on us, but prior to my wife's appointment she showed and put on clean clothes and as we were leaving are dogs jumped on us as they always do. And when we got there Dr. Ho refused to do the procedure on my wife because he said she smelt like dogs. Anyone who has dogs are going to smell like dogs. To say it was unprofessional is an understatement. So needless to say, we'll be finding a more professional pain management Dr. So if you own pets make sure you don't smell like them.

I have a very high tolerance to opiates over the last 40 years. I love Dr. SHAHBANDAR he knows my history included my back surgeries, head on car accident all my other surgeries and work injuries. Been through it all with doc even tried the Neuro stimulator. But I also have Multiple Sclerosis for 27 years very painful in its own right. I had to get the stimulator removed because of this. We tried ALLkinds of opioid medicine and settled on 30 mg ir Oxycodone. Yes a high does but it does not effect me like others with my high tolerance. Then I say Katina Varner and it was fine. Now I see this Hanifa and she was oh oh oh no no no that is way too high a does. So she cut me 30 pills 1 a day. This has been the most painful month I have had in years. 4 am this morning I woke up hurting bad but hav to save them because it gets worse after a hard day. So I eat a VALIUM not made for pain but but but. She did not at all cared what I had to say she cut my meds and walked out. If I was to see Katina all would be great. I am 67 and would love to live somewhat pain free now but with Hanifa doesn't look like it. Why why why was it ok with the actual PAIN DOCTOR that put me on these meds and NP Katina but now with Hanifa I am in pain again. What is wrong with this picture. They are there and have been to help my pai until now. ALL MY DOCTORS know ALLmy medicine I take I hide nothing. So if yo want your pain meds cut down because Hanifa feels you're taking too much without consulting with the ORIGINAL DOCTOR SHAHBANDAR that put me on this good luck to her. She looked at me and said oh ABSOLUTELY NO ONE IS GOING TO GIVE YOU THIS MUCH. Well that is why I have been coming to Dr. SHAHBANDAR For almost 10 years. By far the worst I have felt pain wise in years.

Ms. Waarith was very concerned about my husband's issues. She addressed the issues and gave us alternatives that would take care of the issues. She was very prompt on consulting with his other doctors. He even received a phone call that she had consulted with the other doctors. She was very compassionate about his pain issues. We will definitely come back and see her again.

She doesn't manage your pain, she just makes appointments for the real Doctors to do there job. Dr. Ho is great though.

I met Dr Ho for the first time a few minutes before my procedure. His friendliness was refreshing as we talked about how I could have got the problem. When he said he would “Try” to fix me and I told him I realize that’s all he could do because he wasn’t GOD! It’s a wait and see time for me now.

I was extremely comfortable with my doctor with his nurses the anesthesiologist everyone was informative knowledgeable made things very clear and understandable. This was my 1st time here, and I had a great experience. The office staff were friendly and helpful. The nurse was very thorough and friendly. Ms. Cline was very kind and understanding. She took the time to get a detailed history and explained what steps we would take first. I felt like I was listened to and that my concerns were heard. Julie did a fine job. I just needed her to approve an ablation. An appointment was made for today. Apparently my insurance denied the ablation and my appointment was cancelled. I would've appreciated it if anybody told me it was cancelled after i scheduled a day off work and so did my ride. This is NOT the first time this has happened. I should be credited for my time as I would have been charged if I didn't show for an appointment.

Rude, condescending and never asked for back story of what I’ve been through and diagnosed. This was the FIRST time meeting him. It wasn’t upset during appt about the medications being lowered, I was upset with his method of removing my medicine so quickly which is AGAINST CDC guidelines. Causing me unnecessary suffering with withdrawal at how quickly removing medicines. No matter what I asked or said, he was adamant on doing it his timeline. Even after telling him I’m the SOLE caregiver (lives with mom) for my mother who JUST had DOUBLE MASTECTOMY and now doing chemo. I have to drive her to Chicago for ask Dr appts and chemo while being rapidly taken off medications. He told me, “We aren’t talking about your mother, it’s about you.” “I don’t care about this.” What a dr. & totally concerned about speaking out and retribution like the other np did to me from another office which is why I went to different one. Kept telling me there are other Drs I can go to and I don’t have to stay there. That was repeated at least 4xs.

Yesterday Dr. R Puranik performed a Bil TXTF L4-5. I could barely walk into CPC, walking with a cane in major pain. When I went home I fell asleep until 10:10 PM. When I woke up. I could walk with out my cane! I baked bread, washed dishes, and a few other household duties that I haven't been able to do for months. Dr Puranik and his staff are wonderful!

I will never return. Lack of communication between nurse practitioner and patient is very disappointing. I have never been treated so poorly and a doctor's office. The nurse practitioner had absolutely no compassion and very rude. I have never left a doctor's office feeling like I was accused of using my medicine wrong . I was denied my medication refill, a medication that helped with the pain in my lower back. Not only did I rely on this medication to help with my pain in my back but also with my hip replacements, arthritis in both of my wrists from being broken. I will never go to another younger doctor or nurse practitioner. The only doctors that I will see from now on will be much more knowledgeable, compassionate, and willl sit and listen to you. I was being nice when I even gave the nurse practitioner one star!
