What is ACCESS Program

It is a Medicare approved technology-supported remote chronic care management program — such as lifestyle support, remote monitoring, wearables, coaching, and medication management for one or more chronic conditions.

Better Access to Your Care Team

At Centers for Pain Control, we are committed to making it easier for you to receive the care you need, especially if you are living with chronic health conditions. 

As part of a Medicare-approved care coordination program, we are expanding the ways you can access your healthcare team between office visits. This program is designed to help you stay healthier, avoid unnecessary emergency room visits and hospitalizations, and ensure your care is well coordinated.

Centers for Pain Control is partnering with PsynergyHealth to provide you with coordinated care for your chronic joint, spine, mobility care and behavioral conditions

What This Means for You

You will have access to:

• A dedicated care team to help coordinate your healthcare.
• Assistance with some medications, referrals, and follow-up appointments.
• Support when you have questions or concerns between office visits.
• Easier communication with our office by phone or through the patient portal.
• Shared care plan. 

Who Is Eligible?

Medicare patients with chronic medical conditions.

Is There a Cost?

This is a Medicare-covered service. 
You will be receiving communication from PsynergyHealth with direction on how to enroll in the program.

Your Participation Is Your Choice

Enrollment is completely voluntary. You may choose to participate or decline, and you may discontinue participation at any time.
