I had a very great greatest experience like I always do, but she was she’s gonna step beyond she made to ease my mind and any pain that that I had she was ready to address it right away very kind and very very understanding and I thank her and I think you guys also stay blessed.

She gave me many measurements, careful testing, and went over realistic goal for a plan of care to treat me. Me and my wife has 2 dogs so of course they are going to climb on us, but prior to my wife's appointment she showed and put on clean clothes and as we were leaving are dogs jumped on us as they always do. And when we got there Dr. Ho refused to do the procedure on my wife because he said she smelt like dogs. Anyone who has dogs are going to smell like dogs. To say it was unprofessional is an understatement. So needless to say, we'll be finding a more professional pain management Dr. So if you own pets make sure you don't smell like them.

I have a very high tolerance to opiates over the last 40 years. I love Dr. SHAHBANDAR he knows my history included my back surgeries, head on car accident all my other surgeries and work injuries. Been through it all with doc even tried the Neuro stimulator. But I also have Multiple Sclerosis for 27 years very painful in its own right. I had to get the stimulator removed because of this. We tried ALLkinds of opioid medicine and settled on 30 mg ir Oxycodone. Yes a high does but it does not effect me like others with my high tolerance. Then I say Katina Varner and it was fine. Now I see this Hanifa and she was oh oh oh no no no that is way too high a does. So she cut me 30 pills 1 a day. This has been the most painful month I have had in years. 4 am this morning I woke up hurting bad but hav to save them because it gets worse after a hard day. So I eat a VALIUM not made for pain but but but. She did not at all cared what I had to say she cut my meds and walked out. If I was to see Katina all would be great. I am 67 and would love to live somewhat pain free now but with Hanifa doesn't look like it. Why why why was it ok with the actual PAIN DOCTOR that put me on these meds and NP Katina but now with Hanifa I am in pain again. What is wrong with this picture. They are there and have been to help my pai until now. ALL MY DOCTORS know ALLmy medicine I take I hide nothing. So if yo want your pain meds cut down because Hanifa feels you're taking too much without consulting with the ORIGINAL DOCTOR SHAHBANDAR that put me on this good luck to her. She looked at me and said oh ABSOLUTELY NO ONE IS GOING TO GIVE YOU THIS MUCH. Well that is why I have been coming to Dr. SHAHBANDAR For almost 10 years. By far the worst I have felt pain wise in years.

Ms. Waarith was very concerned about my husband's issues. She addressed the issues and gave us alternatives that would take care of the issues. She was very prompt on consulting with his other doctors. He even received a phone call that she had consulted with the other doctors. She was very compassionate about his pain issues. We will definitely come back and see her again.

She doesn't manage your pain, she just makes appointments for the real Doctors to do there job. Dr. Ho is great though.

I met Dr Ho for the first time a few minutes before my procedure. His friendliness was refreshing as we talked about how I could have got the problem. When he said he would “Try” to fix me and I told him I realize that’s all he could do because he wasn’t GOD! It’s a wait and see time for me now.

Julie did a fine job. I just needed her to approve an ablation. An appointment was made for today. Apparently my insurance denied the ablation and my appointment was cancelled. I would've appreciated it if anybody told me it was cancelled after i scheduled a day off work and so did my ride. This is NOT the first time this has happened. I should be credited for my time as I would have been charged if I didn't show for an appointment.

Rude, condescending and never asked for back story of what I’ve been through and diagnosed. This was the FIRST time meeting him. It wasn’t upset during appt about the medications being lowered, I was upset with his method of removing my medicine so quickly which is AGAINST CDC guidelines. Causing me unnecessary suffering with withdrawal at how quickly removing medicines. No matter what I asked or said, he was adamant on doing it his timeline. Even after telling him I’m the SOLE caregiver (lives with mom) for my mother who JUST had DOUBLE MASTECTOMY and now doing chemo. I have to drive her to Chicago for ask Dr appts and chemo while being rapidly taken off medications. He told me, “We aren’t talking about your mother, it’s about you.” “I don’t care about this.” What a dr. & totally concerned about speaking out and retribution like the other np did to me from another office which is why I went to different one. Kept telling me there are other Drs I can go to and I don’t have to stay there. That was repeated at least 4xs.

Yesterday Dr. R Puranik performed a Bil TXTF L4-5. I could barely walk into CPC, walking with a cane in major pain. When I went home I fell asleep until 10:10 PM. When I woke up. I could walk with out my cane! I baked bread, washed dishes, and a few other household duties that I haven't been able to do for months. Dr Puranik and his staff are wonderful!

When other procedures such as chiropractic, physical therapy, acupuncture..quit helping you have to do something to try and obtain some relief.

He came into the office, did not even introduce himself. Just started in on me because I have three decades of taking the same medication. It's not a joke to me. It's my life told me what he wanted and walked out. That was it

I will never return. Lack of communication between nurse practitioner and patient is very disappointing. I have never been treated so poorly and a doctor's office. The nurse practitioner had absolutely no compassion and very rude. I have never left a doctor's office feeling like I was accused of using my medicine wrong . I was denied my medication refill, a medication that helped with the pain in my lower back. Not only did I rely on this medication to help with my pain in my back but also with my hip replacements, arthritis in both of my wrists from being broken. I will never go to another younger doctor or nurse practitioner. The only doctors that I will see from now on will be much more knowledgeable, compassionate, and willl sit and listen to you. I was being nice when I even gave the nurse practitioner one star!

Ms Velasco is a very great doctor or NP in my opinion DR she very good in what she do when it comes to her patients she very understanding and caring about her patients she going keep coming with solution to solve our issues as patient. I’m a witness to that I don’t have multiple procedures and multiple treatments and I can say from when I first went to see her editing go well because we had ultrasound from a different office where I went to, so she decided to do her own ultrasounds and then once she got those back, we started my procedures we starter my treatments and my legs have got somewhat better than what they have when I first started and for her to be a nurse practitioner she have a lot of knowledge she really care’s for her patience. That’s what I see every Time I see her if it’s in the office for a follow up or if it’s my procedure day she’s gonna always make sure that you’re comfortable while she’s doing the procedure on you. That’s one thing I like about her. She made sure that her patient is comfortable when she get ready to do her procedure procedures on our veins.

I was very disappointed that I was promised pain medication for my lower back and that it would be sent in to my pharmacy for pick up in the evening. It’s over a day later and the pharmacy hasn’t received the prescription. I went to discuss interim medication to hold me over while my pain pump was removed. I’m very disappointed that I was asked several times about having an electrical spinal stimulator implanted instead of my pain pump. I said I wasn’t interested based on comments from people that had it and she asked again. The plan is have my pain pump moved to the opposite side as soon as I heal from the removal.

She was very knowledgeable and answered all of my questions. Made me very comfortable. Extreme satisfied with her. Highly recommend.

Absolutely worst place, completely unprofessional. I was waiting 40 minutes, I asked how much longer and the nurse gave me a nasty look and said your next but if you have to go then reschedule. The nurse had real bad attitude. The surgery place was a dump. The chairs were beat to hell. They are not putting any money into practice. My first appointment amout 2 months ago. a bunch of court reporters were walking in. Obviously someone was suing the practice. Like a said the place is a dump with employees that have bad attitudes. Caution, stay away.

She is always prepared meaning she’s read your file before you see her she’s not afraid to send you for the scans and tests you need I’m super happy with her for a long time I felt I wasn’t being listened to but with her I always know my care will be great

Chelsea Dalton NP took care of the prescriptions i needed for major lumbar surgery on my back, after the hospital, surgeon and Primary Care Physician released me with 2 days of medication on a Friday after 5pm. When I got a hold of my doctor and surgeon they told me to go to pain management. 3 days later I went to Center for Pain Control in Hobart for appointment. This was the first available opening. I was in alot pain. Chelsea Dalton read my discharge papers from the hospital and listened to me. She responded with appropriate prescriptions, Chelsea Dalton had good bedside manners. I highly recommend her.

To reiterate, because my last review might not been understood. My oncologist (my referring Dr) had to give me something to help me somewhat because katina didn't send in a different extended release medication that my insurance would cover after the first one my insurance wouldn't cover. So right now I should have 1x a day extended release pain medication 1x breakthrough pain medication to take per day along with an anti convulsing to take for sleep but I do have thanks to my oncologist who sent me 2 weeks worth or pain meds and anti convulsants. So I have the 1 breakthrough pain med and anti convulsants but not the extended release medication because after 6 days I still haven't got it after the one they sent in Friday after my Thursday visit wasn't accepted by my insurance. So I'm still waiting for that. Thank god for my oncologist or I'd be screwed right now. I have people who depend on me, my father who has many problems, I have to shop for him, cook for him, do housework, do anything he needs and I got a few friends who have no vehicles that I take to go shopping weekly, go to Drs, go to get a few things they need. They all depend on me and if I can't help myself how can I help them. I got physical therapy also weekly. I won't be able to do any of this without help from katina. Thank god for my oncologist who gave me the pain meds, at least I can take the one a day right when I know I gotta help my father, my friends, so they can get what they need or do what they need to do or I can get whatever my dad needs for him because without me they have no one to help them, they depend on me. So hopefully Katina can get me the extended release I'm needing and technically already gave me but I couldn't get it, I don't get the hold up for an alternate ER but this is causing me slot of stress. So hopefully tomorrow I get it finally so I can take it with my 1x a day breakthrough pain medication as it was supposed to be when I talked to her Thursday at my appointment.

I first went here on the February 24th I had to be there at 1:30pm for my 2pm appointment to fill out paperwork. I didn't even get called back til after 3pm waited in room for another 30-45 minutes, then had to wait to check out for another 15 or so minutes. I didn't get to leave until 4:15pm. Its ridiculous! People in pain should not have to wait this long to see someone for their pain. I had to stand, sit, back to standing because I was getting so sore from waiting. Fast forward to April 3rd my follow up after receiving 2 lower back injections on the 17th of March that didn't do much and 2 injections on the 27th for my neck which also didn't work but gave me nausea for 3 days upset stomach for days after, hot flashes, itching of my upper arms and didn't do absolutely nothing for my neck pain. Then seeing her for the follow-up on the 3 of April she called in an Extended Release pain medication that I went afterwards to CVS to see it it was there, they said nothing had been called in so I called back to Dr they said they won't send it out til end of the day, so I asked the pharmacy about the medication I was supposedly gonna get sent the pharmacy said they don't carry it but another location did so I called back to have them send it there instead. Flash forward till around 6pm they close at 5 I called the CVS I gave them to send it to, it wasn't called in Thursday at all, Friday morning I call the CVS they called it in, thank god.. Then I find out my insurance won't cover it so after back and forth between pharmacy, Her office and my insurance I needed to have another ER medication called in that my insurance would cover. I told her office and the lady who does the medications that my insurance won't cover it, there is 2 others ER that they cover, I had my insurance call her also to tell her they won't cover it because it ain't in their formulations etc .. I was told I'd get a call back Friday 2 different times after not getting called back I called back right before closing, I was put on hold and told sit by the phone I would get a call back after the medication lady talked to the Dr. No one called, Monday I called again was told I get a call back their working on it, still no call. They say their trying to get a PA from my insurance for the medication my insurance and I both told them wouldn't be covered,like what the heck, I told them trying to get a PA for it was pointless but was told they had to show they tried, its unbelievable. Now Tuesday I see my oncologist who said she would talk to Katina and Wednesday I got a call from my oncologist saying they asked her if they could give me something to help me. So I got a med to take one time a day for pain and a anti convulsant to take.. The one for pain was a pain med I was supposed to take 1x as I took the extended release medication katina was supposed to call in for breakthrough until the ER kicked in. My oncologist told me katina would call me at the end of the day on Wednesday and guess what no call.. I still have not got the Extended Release medication yet after pretty much a week after seeing her. So tomorrow on Thursday the 10th I'm gonna call CVS see if she sent in the Extended Release pain medication I was supposed to get to take with the 1 breakthrough pill I gotta take per day and the anticovulsant medication. So hopefully after a week ill actually get the Extended Release medication I've been needing. Its so sad my other Dr who referred me to here to give me something to help that I should be getting from katina because that's why I was sent here, to get more help than my oncologist could give me, but so far have gotten no where. So hopefully Thursday ( tomorrow) I can actually get the Extended Release medication I need to take with the others from my oncologist to actually give me some relief. I been struggling so bad since my first appointment here on February 24th and I'm still struggling as of April 9th. I might have to change to the clinic in Munster or Hobart but this one was closer to me (45 minutes) but I might have to and just drive farther to get faster service, not have to wait for hours and actually get help if I don't get any from her today when I call. Or just go somewhere else all together, time will tell.

She doesn't care at all. Who is she to tell me to stop using nicotine before she prescribes pain meds. I have been going to her for almost 3 yrs no pain relief just injections and procedures and therapy. I have osteoarthritis,fibromyalgia, systematic lupus,degenerative disc disease,auto immune and so forth I'm 45 so I have to live the rest of my life in pain with no relief. Not a very good pain management place
