Ms Velasco is a very great doctor or NP in my opinion DR she very good in what she do when it comes to her patients she very understanding and caring about her patients she going keep coming with solution to solve our issues as patient. I’m a witness to that I don’t have multiple procedures and multiple treatments and I can say from when I first went to see her editing go well because we had ultrasound from a different office where I went to, so she decided to do her own ultrasounds and then once she got those back, we started my procedures we starter my treatments and my legs have got somewhat better than what they have when I first started and for her to be a nurse practitioner she have a lot of knowledge she really care’s for her patience. That’s what I see every Time I see her if it’s in the office for a follow up or if it’s my procedure day she’s gonna always make sure that you’re comfortable while she’s doing the procedure on you. That’s one thing I like about her. She made sure that her patient is comfortable when she get ready to do her procedure procedures on our veins.

I was very disappointed that I was promised pain medication for my lower back and that it would be sent in to my pharmacy for pick up in the evening. It’s over a day later and the pharmacy hasn’t received the prescription. I went to discuss interim medication to hold me over while my pain pump was removed. I’m very disappointed that I was asked several times about having an electrical spinal stimulator implanted instead of my pain pump. I said I wasn’t interested based on comments from people that had it and she asked again. The plan is have my pain pump moved to the opposite side as soon as I heal from the removal.

She was very knowledgeable and answered all of my questions. Made me very comfortable. Extreme satisfied with her. Highly recommend.

Absolutely worst place, completely unprofessional. I was waiting 40 minutes, I asked how much longer and the nurse gave me a nasty look and said your next but if you have to go then reschedule. The nurse had real bad attitude. The surgery place was a dump. The chairs were beat to hell. They are not putting any money into practice. My first appointment amout 2 months ago. a bunch of court reporters were walking in. Obviously someone was suing the practice. Like a said the place is a dump with employees that have bad attitudes. Caution, stay away.

She is always prepared meaning she’s read your file before you see her she’s not afraid to send you for the scans and tests you need I’m super happy with her for a long time I felt I wasn’t being listened to but with her I always know my care will be great

Chelsea Dalton NP took care of the prescriptions i needed for major lumbar surgery on my back, after the hospital, surgeon and Primary Care Physician released me with 2 days of medication on a Friday after 5pm. When I got a hold of my doctor and surgeon they told me to go to pain management. 3 days later I went to Center for Pain Control in Hobart for appointment. This was the first available opening. I was in alot pain. Chelsea Dalton read my discharge papers from the hospital and listened to me. She responded with appropriate prescriptions, Chelsea Dalton had good bedside manners. I highly recommend her.

To reiterate, because my last review might not been understood. My oncologist (my referring Dr) had to give me something to help me somewhat because katina didn't send in a different extended release medication that my insurance would cover after the first one my insurance wouldn't cover. So right now I should have 1x a day extended release pain medication 1x breakthrough pain medication to take per day along with an anti convulsing to take for sleep but I do have thanks to my oncologist who sent me 2 weeks worth or pain meds and anti convulsants. So I have the 1 breakthrough pain med and anti convulsants but not the extended release medication because after 6 days I still haven't got it after the one they sent in Friday after my Thursday visit wasn't accepted by my insurance. So I'm still waiting for that. Thank god for my oncologist or I'd be screwed right now. I have people who depend on me, my father who has many problems, I have to shop for him, cook for him, do housework, do anything he needs and I got a few friends who have no vehicles that I take to go shopping weekly, go to Drs, go to get a few things they need. They all depend on me and if I can't help myself how can I help them. I got physical therapy also weekly. I won't be able to do any of this without help from katina. Thank god for my oncologist who gave me the pain meds, at least I can take the one a day right when I know I gotta help my father, my friends, so they can get what they need or do what they need to do or I can get whatever my dad needs for him because without me they have no one to help them, they depend on me. So hopefully Katina can get me the extended release I'm needing and technically already gave me but I couldn't get it, I don't get the hold up for an alternate ER but this is causing me slot of stress. So hopefully tomorrow I get it finally so I can take it with my 1x a day breakthrough pain medication as it was supposed to be when I talked to her Thursday at my appointment.

I first went here on the February 24th I had to be there at 1:30pm for my 2pm appointment to fill out paperwork. I didn't even get called back til after 3pm waited in room for another 30-45 minutes, then had to wait to check out for another 15 or so minutes. I didn't get to leave until 4:15pm. Its ridiculous! People in pain should not have to wait this long to see someone for their pain. I had to stand, sit, back to standing because I was getting so sore from waiting. Fast forward to April 3rd my follow up after receiving 2 lower back injections on the 17th of March that didn't do much and 2 injections on the 27th for my neck which also didn't work but gave me nausea for 3 days upset stomach for days after, hot flashes, itching of my upper arms and didn't do absolutely nothing for my neck pain. Then seeing her for the follow-up on the 3 of April she called in an Extended Release pain medication that I went afterwards to CVS to see it it was there, they said nothing had been called in so I called back to Dr they said they won't send it out til end of the day, so I asked the pharmacy about the medication I was supposedly gonna get sent the pharmacy said they don't carry it but another location did so I called back to have them send it there instead. Flash forward till around 6pm they close at 5 I called the CVS I gave them to send it to, it wasn't called in Thursday at all, Friday morning I call the CVS they called it in, thank god.. Then I find out my insurance won't cover it so after back and forth between pharmacy, Her office and my insurance I needed to have another ER medication called in that my insurance would cover. I told her office and the lady who does the medications that my insurance won't cover it, there is 2 others ER that they cover, I had my insurance call her also to tell her they won't cover it because it ain't in their formulations etc .. I was told I'd get a call back Friday 2 different times after not getting called back I called back right before closing, I was put on hold and told sit by the phone I would get a call back after the medication lady talked to the Dr. No one called, Monday I called again was told I get a call back their working on it, still no call. They say their trying to get a PA from my insurance for the medication my insurance and I both told them wouldn't be covered,like what the heck, I told them trying to get a PA for it was pointless but was told they had to show they tried, its unbelievable. Now Tuesday I see my oncologist who said she would talk to Katina and Wednesday I got a call from my oncologist saying they asked her if they could give me something to help me. So I got a med to take one time a day for pain and a anti convulsant to take.. The one for pain was a pain med I was supposed to take 1x as I took the extended release medication katina was supposed to call in for breakthrough until the ER kicked in. My oncologist told me katina would call me at the end of the day on Wednesday and guess what no call.. I still have not got the Extended Release medication yet after pretty much a week after seeing her. So tomorrow on Thursday the 10th I'm gonna call CVS see if she sent in the Extended Release pain medication I was supposed to get to take with the 1 breakthrough pill I gotta take per day and the anticovulsant medication. So hopefully after a week ill actually get the Extended Release medication I've been needing. Its so sad my other Dr who referred me to here to give me something to help that I should be getting from katina because that's why I was sent here, to get more help than my oncologist could give me, but so far have gotten no where. So hopefully Thursday ( tomorrow) I can actually get the Extended Release medication I need to take with the others from my oncologist to actually give me some relief. I been struggling so bad since my first appointment here on February 24th and I'm still struggling as of April 9th. I might have to change to the clinic in Munster or Hobart but this one was closer to me (45 minutes) but I might have to and just drive farther to get faster service, not have to wait for hours and actually get help if I don't get any from her today when I call. Or just go somewhere else all together, time will tell.

She doesn't care at all. Who is she to tell me to stop using nicotine before she prescribes pain meds. I have been going to her for almost 3 yrs no pain relief just injections and procedures and therapy. I have osteoarthritis,fibromyalgia, systematic lupus,degenerative disc disease,auto immune and so forth I'm 45 so I have to live the rest of my life in pain with no relief. Not a very good pain management place

For a NP Chelsea Dalton she is very knowledgeable and very helpful when it comes to my care and or about any questions I have. She doesn't push me into doing or having any procedures that are not necessary. And she explains in detail up coming procedures to make me comfortable going into it. Number one, Dr. Ho was never there! All they wanna do is run you around to do a bunch of procedures that you really don’t need so they can charge up your insurance !!! It’s all about the money $$$$$ And the doctor was rude, and the staff lied a lot !!! This place is trash In my opinion !!! Buyer Beware!!!!

Christy was great. She addressed my questions, took time to explain what the tests found, and offered solutions. In addition she went above and beyond helping me get a procedure scheduled on the other side of the practice.

Dr.shahbandar has again met my expectations. It’s only been 24 hours since my procedure but I have experienced a little relief. My procedure usually can take up to 2 weeks to experience improvement. He explained the procedure and told me what to expect. His staff were also great…you could tell that they cared. He has a great personality unlike some physicians who feel that you’re too ignorant to understand what they’re doing.

Almost all the doctors have left , Every time I asked for a doctor the NP gave me a run around wont let me talk to him. She said she would call PT and never did. then Told me I will call today and I told I don't trust her or anything she says. Asked for a doctor told me she talks to doctor to review things and that should be good enough. When I saw her delete things on computer last visit should have given me heads up then Billed Medicare as if I saw a doctor no longer there since December. called to report her to manager they said we cant see you no because I refused to believe the nurse . sat in room 39 minutes and I started to leave they rushed me back to room I should of kept going home. The staff told me it going down hill fast last time I was there I should have just started looking then. RUN AWAY PEOPLE

My regular location is in Munster but came to Hobart because Munster didn’t have my time requirements. This was the first time I met Dr Shahbandar. He introduced himself knowing it was my first time with him. The injections in my right shoulder was almost painless and I returned to work afterwards. I would definitely return again.

The knee injection before this one ( sometime back in July ) was much better I got instant pain relief and it lasted longer.Not sure why ,,hard to get answers,,about this one

You cannot discuss anything with her and if you do she has an attitude about it.

I never saw a physician, apparently the physicians were too busy to bother with me. I I saw a NP twice who advised me that they wanted to do invasive and expensive alternative treatments and take me off the pain meds that have sustained me for 18 years. In those years prior, I've had injections, PT, Tens, and consulted surgeons. But, it has been the medications that worked. We have reduced those meds to the minimum Effective doses but that's not enough. They can't make much money by simply prescribing what works, they need to have surgery and to wire my spine to make the most, then she told me me CT scan doesn't look bad enough to justify using medications, but it looks bad enough to justify surgery and implanting electrodes and a controller in my back. Anyone who seeks their help for pain, will be disappointed unless your willing to complete their useless and expensive procedures.

I'am in more pain now, than before. A new pain that goes down my right hip and thigh.

She answered all of my questions and even proactively handled a prescription conflict that prevented the confusion that typically ensues when we relocate each December. The office looks run down. The chairs are HORRIBLY uncomfortable. The staff is slow at assisting any person. There is no reason any patient should wait an hour and 15mins. Once I was in the room with the physician, I felt immediately dismissed because of my "yellow file", which apparently flags Medicaid patients in this office. He didn't care to talk about my pain, just said I'll need PT for compliance. Essentially I felt like I was treated like a drug seeking patient. This physician never looked at the MRI I had (ONLY A WEEK OLD) only asked for more tests, and completely ignored me as a person. As if that wasn't enough, I explained to the Dr., that I was pretty certain I was going to need another knee surgery based on the results of that MRI, and how much pain I was in. I was offered a prescription that I ex. plained is not effective for me. I went to fill prescriptions and was extremely surprised to have one for the drug I said was ineffective and Narcan. I found this shocking and insulting, especially because it was never suggested this would be added to my medication list. I left this office in more pain than when I came in!!! This office and staff is a sad display of professionalism. The staff, including the Dr., should consider their personal conversations near by patient rooms, as they are not well insulated for soundproofing.

I had to wait 40 minutes before being placed in a treatment room. I was then informed by the staff that I would have to wait another hour or more before being seen. Once I was seen she had no idea as to what to do. She stated she would reach out to the Dr. for help and get back in touch with me for which she never did.

I felt decent after I went home. I cleaned my work area, made a big dinner, and around 8pm I knocked out. I slept like a baby. Today I’m sore. But I’m still trying to keep busy.

I needed help with my medication and change it instead of let me come in and talk to him and help me change it. I told him that I did not want to take the strong medication no more and I disposed of it they would not give me no more medication they wouldn’t let me come in and explain my situation, they drop me very very bad. Doesn’t care about the patient at all.

Christy Kersey Is great! She took the time to explain in detail the results of my ultrasound for my legs. She Asked questions and listened to my answers, Even questions I didn’t realize pertained to the problem I’m having. We have a plan to take care of my veins to fix cramping and heaviness in my legs, and to avoid future risks.

Dr Shebandar is a great doctor who actually really cares about his patients. He takes his patients very personal. I trust him and I am thankful to be under his care. He always asks me about my life and my activities so he can understand what is going on and what makes me hurt less or more. I am just very happy to have him as my doctor and great staff. Portal works great. Saves time. I can ask questions, make any payments through it so the portal works great. Plus I can do my pre check in on it so when I go to my appointment I can go right in.

Julie Stein truly researched my pain issues. Concluded multiple causes. Julie methodically did her du-diligences in order to help me with my pains. Not all pains are the same. Julie's is very experienced which also makes her very knowledgeable and Julie understands how to best apply these skills. I for one am thankful for her choice of professions. Dr Tarek SHAHBANDAR is a great doctor but the new system isn’t working for the good of the patients you run out of medication before your next appointment and it takes them forever to get your medication even after you see the doctor

Doc Shahbandar just merged with the other Pain Clinics so wait times are a bit long but that is to be expected and hopefully get back to normal. As far as Doctor himself I trust my back to him. I've been literally through every procedure, PT,medication opiate and non-opiate, 3 Failed lumbar fusions NOT BY SHAHBANDAR. Also have had a Neuro stimulator implanted which worked great for the pain but Agrivated my MS (Multiple Sclerosis) severe enough Doc removed the stimulator. Have been through numerous PainClinics for 37 years. Have been with Doctor Shahbandar the last 10, I believe and he is by far thee BEST ALL AROUND DOCTOR ❤️. His crew is also PHENOMENAL in making you feel confident they know what they are doing. Give Doc Shahbandar a shot, at least try him once you will not be looking for another Doctor I guarantee you.

Although it’s very hard to have someone call you back and everything is computerized I would suggest to try and have someone too speak too at times of importance

Dr. SHAHBANDAR listened to what I had to say and understood the type of pain that I am having. He also told me that he was pretty booked but if I wanted to get relief sooner, I could have Dr. Ho do the injection. I appreciated that he was concerned about getting me relief sooner.

Dr Shahbandar is excellent in what he does. He takes time with you and explains things to where you understand. On the other hand the phone system stinks. It takes forever to talk to someone in the office. You go through several steps just to get put on hold for a long time. What ever happened when you called the office you got to talk to someone right away ?
